New York, NY - NYC Health + Hospitals announced today that it will open a second Bridge to Home facility in Brooklyn, significantly expanding its transitional housing program for unhoused New Yorkers who have been diagnosed with serious mental illness. The new site builds on the success of the original facility, which has provided a stable, supportive environment for patients leaving hospital care.
The Bridge to Home program is designed to bridge the gap between inpatient psychiatric treatment and permanent housing. Many individuals experiencing homelessness and mental health challenges face a cycle of emergency room visits and street homelessness because they lack a safe place to recover. The new Brooklyn facility will offer private rooms, on-site clinical support, and case management services to help residents stabilize and transition into long-term housing.
Officials say the expansion is a direct response to the growing need for integrated health and housing solutions in the city. By providing a structured, therapeutic setting, the program aims to reduce hospital readmissions and improve overall outcomes for some of the city's most vulnerable residents. The new facility is expected to begin operations later this year, adding dozens of beds to the existing capacity.
