Record cyclosporiasis outbreak tests the response of a 'weakened' health system

A record number of cyclosporiasis cases this year is putting serious pressure on public health agencies, just as many of those agencies are still dealing with staffing and budget reductions from the previous administration. The parasitic infection, which spreads through contaminated food and water, has surged to levels never seen before in a single season, and the response has been anything but smooth.

State and local health departments are the first line of defense, tracking down sources of the parasite, interviewing patients, and coordinating with food safety inspectors. But those departments rely heavily on federal funding and technical support, and that support has been thinning. The Trump administration's cuts, which took effect last year, reduced the workforce at agencies like the CDC and FDA, and also trimmed grants that help states do this kind of investigative work. Now, with case numbers climbing, those gaps are showing.

The result is a patchwork response. Some states are managing, but others are struggling to keep up with the volume of interviews and lab tests. The CDC has issued guidance, but without enough staff to analyze data or trace outbreaks in real time, the picture of where the infections are coming from remains incomplete. Food safety experts say the timing could not be worse, because cyclosporiasis outbreaks often involve imported produce, and tracing those supply chains takes time and coordination.

For now, the public is advised to wash produce thoroughly and be aware of symptoms, which include severe diarrhea, cramping, and fatigue. But the bigger concern is long term. If the system stays weakened, the next outbreak, whether it is cyclosporiasis or something worse, will be even harder to contain.
