A growing number of radiology practices are taking artificial intelligence into their own hands. Rather than waiting for outside vendors to deliver finished tools, these groups are building and deploying AI systems internally. The result is a shift that makes the boundary between technology development and everyday clinical work increasingly hard to see.

This approach, sometimes described as "AI-native," marks a departure from the usual pattern. In the past, software arrived from external companies and was fitted into existing workflows. Now, radiologists and data scientists sit together, testing models on real scans and adjusting them based on what happens in the reading room. Feedback loops that once took months can close in days.

Supporters argue that in-house development gives practices more control over accuracy, privacy, and the specific needs of their patient population. A model trained on local data may perform better on local cases. It also lets clinicians flag problems quickly and push fixes without waiting on a vendor's release schedule.

Critics point to the cost and complexity. Building reliable AI requires engineering talent, computing power, and careful oversight, resources that smaller practices may lack. There are also questions about regulation, liability, and how to validate tools that never go through a traditional commercial review.

Even so, the trend is gaining momentum. Practices that market themselves as AI-native say the label signals more than technical skill. It reflects a willingness to treat innovation as part of clinical care rather than something separate from it. Whether that model spreads widely may depend on how well these early adopters manage the risks that come with building their own tools.
